Why are some duracell rechargeable batteries black and some green?

I think the black are older. I recently ordered green and got black. They do not hold their charge and I discovered after 30 days. Amazon would not take back. I will not buy rechargeable batteries from them in future.

The ion speed 1000 charger is listed with 4 aa and 2 aaa. the photo and product description seem to suggest it comes with 6 aa and 2 aaa. which is it?

There are 4 shorter slots for the AAA batteries + 4 longer slots for the longer AA batteries. You have to put multiples of 2 of the batteries in to have it charge - so 2 or 4 of the AA - same w/ the AAA (2 or 4).

If I have 1 dead battery (0%) and 1 low battery (25%) and put both in the charger at the same time, will it only make 1 fully charged, or both?

Both will be fully charged you can put one dead and one fully charged on the same circuit and the one that's dead will charge and the other one will stay charged
