does this really works ?

Yes, I have always had a lot of trouble with my ears not equalizing when flying, and it really does help to equalize the pressure in my ears! I usually chew gum also, so that will help me to swallow, or I get some water in my mouth. The swallowing action is important. I just took a 6 hour flight to Hawaii, and I was nervous about the descent. This EarPopper really helped me. I will never get on another airplane without it. I even bought an extra one, as a spare, just in case!

Does this work for fluid behind the eustachian tubes?

I have fluid in middle ear. The device does pop my eardrums but my eustacian tube remains blocked so the fluid can’t drain. So far I have not been able to clear the tube to drain.
