does this come with a shallow spool as shown in the picture?

It only comes with a "standard" spool that takes 620m of 0.33 line. A shallow spool could be bought which will take 300m of 0.30 line. Hope this helps.

Does the reel actually weigh 1.5 kg as stated twice in the advert?

Just weighed one of these reels fully loaded with as much 15 pound line as it will take and a 60 pound shock leader, it came in at approx 900 grams, no where near 1.5 Kg

Is this real left and right handed?

It can be both. The reel arm can be removed and placed left or right. I'm a lefty so it was great it had this option.

Would the spool fit 300m of 0.18mm braid to rim, or need backing as well?

Will definitely need backing as the standard spool is rated for 620m for 0.33 line. Even the shallow spool that could be bought separately is rated for 300m for 0.30 line. Hope this helps.
