Is this opener only good for young coconuts?

Yes, it is.  As a matter of fact, it is mainly good for those young coconuts usually purchased at the store that come already pre-cut and with surround wrap all around.  This way, the tool does not have to do much work as you would just it at the top of the coconut and then hit with the hammer and then move the tool back-and-forth and the whole top will come off, allowing you to eat the coconut pulp.  By the way, I purchased this tool once and it came defective.  I gave it a second chance and it works well.  Now, if you are interested in just drinking the water and not so much on the pulp, there is another tool called CocoDrill, Young Coconut Opening Tool (about $7.50 on Amazon) that works very well as well as some others.  There is also a tool to drill any coconut for the water (not the pulp) called MetalCoco (about $16 on Amazon) that works with not just young coconuts.  All in all, in general, most them are designed for young coconuts.   Hope this helps.

Does the coconut have to be stripped of the outer "skin", the green part, for this to work?

No, I had info from Asian store.  I did this easy...  No stripping, just put the medal part on top and use malat to hammer it down the medal cuts and then pull the top to enjoy.                  I looked at youtube videos, to see.   Very easy, for someone not strong like me.   (safe and good tool)
