How thick are the small/mediums? How much do they compress? Can they be worn under athletic shoe insoles, or less effective that way somehow?

They are really great if you have heel sensitivity or damage. I would definitely put them on top of the insoles because you want to have that gel right there underneath your heel. Hard to say how thick they are, but I did a search to try and find the thickest ones I could. They’re all about the same. I suppose you could put one on top of the other to double them if you wanted a little thicker. I was in a Velcro boot for 12 years with heel nerve damage, and I’m finally able to walk again with two shoes on because of these gel heel inserts. Worth a try!

do these have adhesive on the bottom so they can be worn with a backless shoe?

Yes, there is adhesive on the bottom of each heel pad. You peel off a protective cover to reveal the adhesive. That being said, they may not be suited for backless shoes if you are moving about a lot. They are perfect for regular shoes, but if you use backless shoes you may need to combine additional adhesive from another source.
