Q: is the guitar setup professionally before shipment?

It depends on your definition and personal preferences, actually. As PRS themselves often note, the guitar will be ready to play because they set them up to be so. Mine was certainly ready to play with a very good setup. However - if you're a person who likes their action [fairly] low you may find the factory setup does not suit you. Based on feedback I have heard from others, this is frequently the case. The guitars are definitely ready to play, but the setup may not be what a given individual prefers because the factory setup is with an action height some will consider too high. Must add this here - I ordered one of these and the pickup selector switch was defective. Amazon had a new one to me in short order and the return could not have been easier. The switch on the 2nd guitars works fine. Great customer service. Great guitar.

Q: Is the tremolo arm included?

Yes, along with the associated set of tools.

Q: Is the guitar damaged at all?

Mine had no visible damage to the guitar body, neck, etc. My first one did have a defective pickup selector switch, but I returned it and the replacement worked perfectly (replacement guitar, not switch). Very nice guitar.
