No, the lifts are plastic . I use it on a rug with problem of slipping.

Where can I buy additional risers?

You May want to contact the manufacturer but I don’t think you will need them when they are all on. It may not be stable enough if you tried to make it higher than what it comes with.

Can this unit be adjusted higher than 12" with additional pieces?

The additional risers are 2 inches thick each so the highest you could get the platform off the floor is 4 inches.

Can this be used to set outside a door to step down on?

It is all plastic and could be used outside. Not too attractive, however.
