Do the larger pads have to be spaced further than one inch apart?

They should bc the take up more muscle space

I use a small amount of rubbing alcohol on a paper towel.

does this include a user manual?

The unit comes with a user manual. The replacement pads do not, but are identical to those that come with the tens unit. My back is much better and I keep the unit at the ready for next time.

Do you sell just the small pads separately?

May be they can do... but I think big pad is also very helpful...
