It fits just like a wrist band. It's Velcro so you tighten it up for your comfort.

Do earphones get in the way when running with this on and having earphones plugged into the phone?

Depends on the length of the earphone cable, the longer the cable the better. I would suggest getting a pair of Bluetooth earphones. There are several good quality earphone on Amazon that are reasonably priced and offer great sound quality, Hope this helps.

Does this work for women with a somewhat small wrist?

Basically the entire wrist strap has velcro, so it will tighten down and fit small wrists. It even fits my 11 year old daughter's wrist without slipping around.
