What size should I order for exactly an 18 inch opening?

You have to measure the inside width of your cabinet opening at the inside of the face frame to know which product to order.

Does this include the cabinet door?

I have a 15” wide 18” deep and 21” opening. What do I order?

Your best bet is to go to the rev-a-shelf website and click "Shop By Size" they have a great search tool that will show both the part number that fits perfectly, and all the parts that you could adapt to fit. I ended up finding a part number that was inexpensive on Amazon but I had to build some custom siderails out of 2x4 stock.
