Can this be used on an outdoor wreath?

Yes. But it would be hard to cut. It's a medium square shape. Stones are scattered on the sheet. So cutting isn't easy. Would be easier to get a ribbon and apply hotfix rhinestones to that ribbon.

Is this permanent adhesive? Or should i add my own glue for better adhesion?

The product can be pasted directly after removing the backing paper. It is not recommended to use it with glue.
