Does it come with back screw and gaskets?

Yes it comes with all that. All you need is the hole to put it in!

Fits 3 gal containers purchased around 2005? How do you remove the spigot? I don't want to force it open and break it. Mine is dirty but still works.

Look on the inside where the spigot comes in.The plastic piece is a hex retaining nut use Cresent wrench and unscrew it counterclockwise while holding the outer piece.The nut will screw of and the spigot will pull out.
