Will the balls fit insdie the mouth of a 15 month old child?

Yes! My child put one of the balls in her mouth at 10 months. We had to take it away. Now we just let her play with the xylophone and hammer part.

how can I purchase extra balls?

You can purchase a replacement set (Current price $10 (Prime) here: https://www.amazon.com/dp/B01M04YTFG I actually found that the balls which fit my twin nephews' other ball drop toys fit this toy. I purchased these for them and they LOVE that they shake and add an extra dimension to the noise of this toy. The only drawback is that these don't have "green" and they are plastic. Currently they are $8 (Prime) for a set of extras. They do hold up very well though considering the twins fight over them constantly and bang them around with the hammer and any other toy they can find.
