Can i use this for my four month old?

The spout flow is generally recommended for babies 6 months and older, and the cup shown here is recommended for babies 12 months and older - simply because it is 10 ounces and often 4 months old can't drink that much at a time, and the convenient carrying clip for on-the-go would do a 4 month old no good.

Can I order a replacement nipple?

Yes. Search for Nuk silicone replacement spout, and it will fit this supply cup.

How easily does the lid pop off? my child throws his cup and the current lid pops open

It is a screw top, but never used due to spout impossible to drink any liquid from. Hole too small. Would not recommend purchasing.
