Does this pen turn the side of the left hand black when a left handed writer uses it?

I am left handed and love it. I write normally and not with my arm bent like some left handed people do!

will this write on transparent sticky notes?

I did not have a transparent note to test this pen on but I did try a sticky shipping label and it wrote on that but it did need a second to dry without smearing. I also tried a sticky tab I use for making notes in a book and the results were not very good

Will this ink bleed/run if water drips onto the page?

I had some notes written on a notepad and some condensation from water bottle dropped and ink did not run or spread

will this pen take other uni refills if I prefer 0.38mm for instance?

No. However Uni did make a 0.38 version of this pen in Japan and those refills should be more widely available soon. The 0.38 version only recently came out with different color schemes
