Does this item come with the push button fob?

No, it doesn't come with the button part if the key fob. It also doesn't come with the metal key part. This is only to replace the plastic encasing for those parts, and the screw. It also comes with the mini screwdriver you need. The key and button portions are much more expensive.

Does it work with a 2011 honda civic coupe?

Check the markings on your fob, our 2009 civic Lx coupe has: N5F-S0084A, which this product lists as conformant.

Any suggestions as to how to remove key blade from the plastic it's embedded in? I need to move the key blade into the new shell

If you've got it apart already, there's a set screw that is holding that little piece of plastic to the key blade. The screw might not be visible, but if you didn't take it out, it's still in there. You could always just use snips or dykes to cut that piece of plastic apart and then it slides down the key blade.
