There is a 3 wheel and a 4 wheel for the same price. Why would you choose one over the other?

4 wheels would be more stable then 3. If you can find a 3 wheel with a 360' swivel option, that's the way to go.

Adjusted alignment as instructed, cart still pulls to the right. Should both alignment screws be fully tightened?

Maybe a manufacturer’s error. I put the cart together and there have been absolutely no issues. I use it 4-5 times per week as I am retired. I would contact the the company

Is this mostly metal or cheap plastic?

The frame is metal and there are various plastic parts but the over all construction is high quality. Nothing about this cart is cheap. An excellent value.
