B12 is really the one (and only) thing that should be monitored for people on all (vegan) or mostly plant-based diet. As one ages, B12 intestinal absorption decreases (regardless of diet. I monitor my serum B12 level 1-2X per year. In the past I used a B12 supplement including spray sporadically, and my B12 levels have always remained at the lower end of 'normal.' More recently I took a multi-vitamin pill with far greater dosages of B12 once a week; it works, but the intestine can only absorb a small percentage; such pills come with lots of other vitamins I don't need or want; and they taste terrible! From my research (as a physician), I have determined sublingual B12 as methylcobalamin from a plant-based source offers the most advantages in terms of absorption, efficacy, and sourcing ethics.
