Q: Does this table have to be assembled, or does it come with all the parts attached?

Mine came all assembled. It's a very nice extra table.

Q: Is the tabletop food grade? Meaning non-toxic and can roll dough on it etc...?

Not sure about food grade. We use the table at church when we are having carry in dinners, but we don't usually put the food directly on the table such as dough. I would not even know how to check. Put a cover over the table. It would be solid enough to do things like rolling dough. Hope that helps.
